Picking up from the third part of the series: After Mildred and Betsy have the "emotional" talk, they go to the house of the woman who financed the hospital and tell her about all the shady things Dr. Hanover has been doing at the hospital. Hanover is fired and Nurse Betsy is appointed temporary director of the hospital.
Hanover leaves and takes his patient Charlotte with him to continue with the treatment since it was working, but, something happens and he has to lock her in a closet, which was a terrible decision, since doing so revived her trauma and all her personalities are unleashed. One of Charlotte's personalities ends up murdering him, but when her crisis is over she calls Mildred to tell her everything that happened.
Mildred arrives on the scene and upon seeing the terrible scene helps her escape. At that moment she takes advantage of the fact that Hanover is dead to cut off his head so she can take it to Mrs. Osgood, as she wanted; so she pays her and then gives the head to her son Henry, who is very unhinged and manipulates one of her employees to end his mother's life.
Henry wanted to keep everything, but his mother leaves half of her fortune to charity and the other half to his monkey Petunia; he is left nothing and asks that he be placed in the care of a psychiatric institution where he will spend the rest of his life.
On the other hand, Mildred goes to see Gwen to make the passes, she tells her that she loves her and wants to be with her for the rest of her life, but Gwen reveals to her that they discovered that she has cancer.
A month later Edmmund is finally going to be executed and not by euthanasia as planned, but by the electric chair. Mildred does not want this so she goes to talk to the governor to euthanize him, but he does not listen to her; therefore, Mildred asks Betsy to let him kill him with euthanasia to which she agrees. Because of this, Mildred decides to give her brother a last supper.
Unfortunately nothing goes as they plan because Charlotte appears and in the middle of a crisis, she starts acting like Dr. Hanover, kills Huck and also frees Edmmund. Edmmund asks Betsy to tell him what Mildred's plan was, she tells him and ends up running away with Charlotte.
Two years later, Mildred and Gwen are living a quiet life in Mexico, Gwen's health is better since the cancer treatment is working, everything seems to indicate that they are happy and have a good relationship, but despite this and that the nurse Betsy went to visit them for vacation the three Mildred was always anxious with the idea that her brother was on the run, because she knew he was capable of murdering her.
The dreaded day arrives and Edmmund communicates with her and warns her that he already knows where she is and that he is going to look for her to kill her.
